Do you have a valid TEXAS Drivers License?
Have you ever had an "at fault" acccident while on the job?
Have you ever been injured on the job?
Are you prevented from becoming lawfully employed due to immigration status?
Are you currently employed?
If so, may we contact your current employer?
Have you ever been arrested?
Do you have any felony convictions or misdemeanors?
Are you currently on bail?
Do you have any lawsuits pending?
Are you currently on probation or parole?
If yes, are monthly visits required?
Are you currently receiving unemployment?
Are you currently receiving workers compensation benefits?
Do you own or have access to reliable transportation to work?
Do you have a CDL (Commercial Drivers License - Class A or B)?
If no, is there there anything preventing you from obtaining a CDL, such as a DWI?
Is there anything on your driving record that may prevent you from being added as a driver on the company insurance policy? (ie: DWI, 3+ speeding tickets)
Have you ever been convicted of a DWI?
Have you ever operated any specialty equipment, such as a vacuum trucks, concrete boom trucks, cranes or foundation drill rigs?
Are you willing and able to travel?
Do you have any physical or mental limitations or medical conditions that may prevent you from performing manual and physical labor?
Do you understand that if employed at M&D, you will be working in the elements, such as heat, cold, rain, snow, mud, etc?
Are you willing to work in these conditions?
Are you willing to submit to a drug test?
Do you have friends or relatives working for M&D?
